Please print the contract, fill in what you are looking for, and your name and address and signature.  Any balance owing when you pick up your pup I ask be paid in cash, Money Order or Cashiers Check.   You can mail the completed form back to me.  As soon as a pup or pups meeting your specifications are born I will contact you, and we can go from there.  

 

If you choose to wait for a different pup you have that option as well.
